Where can I obtain a copy of the conversion/exercise form?

To download the conversion/exercise form:

1. Visit the Share Registry and IPO (MY) website at https://srmy.vistra.com.
2. Under the “Investor Services” category, select “Others.”
3. Click on “Form for Download.”
4. Use the drop-down menu under “Company Name” to select your desired company.
5. The relevant form will be displayed for download.
