How do I submit my MITI application via Share Registry and IPO (MY)?

Log in to https://srmy.vistra.com using your email address and password.
Select the relevant MITI Application exercise. Note: If you are not an entitled holder, the MITI Application will not appear in your selection list.
Read and agree to the Terms & Conditions and Declaration.
Tick (✓) the CDS account you wish to use for the MITI Application.
Enter the number of shares you wish to apply for (minimum of 100 shares, in multiples of 100).
Indicate your occupation.
Review and confirm your application details.
Proceed with the payment, which includes the share application amount and the handling fee, via the online payment gateway (FPX or Maybank2U).
